Clinicians Address Marijuana Use with Their Patients

Orange County, CA - September 25th, 2018 -  Cannabis, also known as marijuana, is a psychoactive drug from the Cannabis plant used for medical or recreational purposes. The two most active components in marijuana are tetrahydrocannabinol (THC ), and cannabidiol (CBD).

THC is the chemical responsible for most of marijuana's psychological effects. According to the National Institute on Drug Abuse (NIDA), it acts much like the cannabinoid chemicals made naturally by the body. Cannabinoid receptors are concentrated in certain areas of the brain associated with thinking and memory. CBD is the non-psychoactive cannabinoid that reduces symptoms in patients suffering from seizure and spasm disorders such as epilepsy, and multiple sclerosis.

Marijuana is one of the most widely used substances among pregnant women in the United States. A new American Academy of Pediatrics (AAP) clinical report recommends that pediatricians advise women of reproductive age to refrain from marijuana use while pregnant or breastfeeding due to potential negative consequences to the fetus, infant, or child.

The report shows that cannabis compounds rapidly cross the placenta, and can lead to unfortunate pregnancy outcomes including low birth weight, premature birth, small head circumference, small length, and stillbirth. In addition, other studies suggest that cannabis compounds can also be transferred to the offspring through breast milk, leading to poor pediatric outcomes, such as memory deficits, attention span issues, impulse control, and school performance.

Due to the legalization of medical or recreational marijuana use in more than half of U.S. states, more women are using marijuana while pregnant and breastfeeding. Data showed 36% of women reported they used marijuana at some point in their pregnancy, and 18% reported using it while breastfeeding.

“The fact that marijuana is legal in many states may give the impression the drug is harmless during pregnancy, especially with stories swirling on social media about using it for nausea with morning sickness. But in fact, this is still a big question. We do not have good safety data on prenatal exposure to marijuana. Based on the limited data that does exist, as pediatricians, we believe there is cause to be concerned about how the drug will impact the long-term development of children,” said lead author Sheryl Ryan, MD, chair of the AAP committee on substance use and prevention.

The clinical report discusses how cannabinoids are able to cross the placenta, and affect fetal development. There are also concerns about pregnancy outcome, and long-term consequences for the infant or child exposed to marijuana before birth.
